When should I arrive to my appointment?If you are a new patient we encourage you to come 30 minutes early to fill out new patient paperwork. If you have already printed and filled out the paperwork, or are a returning client we encourage you to arrive 10 minutes early to your appointment.

Do I need to bring anything?We encourage all new patients to bring the following: 1. Your prescription 2. Insurance card(s) 3. Driver's license 4. New patient forms (if printed)

What should I wear to my appointment?Patients are encouraged to wear comfortable clothing. We have shorts and gowns that can be provided for use during your treatment.

Billing/Payments QuestionsAll patients' co-pays are to be paid on the same day of treatment session unless other arrangements are made with the receptionist. Please inform us if you have made any changes in your address, name, phone number, or insurance carrier. The patient is ultimately responsible for all outstanding balances.

Workers' Compensation Patient QuestionsWe appreciate your full cooperation in attending all scheduled therapy sessions. We are required to inform your workers' compensation adjuster and/or rehabilitation case manager of all missed or canceled appointments. It is also required that all missed visits be rescheduled.
